ACL Injuries in the GAA: What the Research Actually Shows

Ask any GAA supporter and they’ll rattle off a list of players sidelined by cruciate injuries without missing a beat. ACL injuries in the GAA have become one of the most familiar storylines in Gaelic games…Mayo’s Paddy Durcan, Tipperary’s Conor Sweeney, Limerick’s Nickie Quaid, Armagh’s Aimee Mackin, Kerry’s Síofra O’Shea, the list of inter-county players who’ve been through ACL reconstruction is long, and growing. But how common is it actually, what happens during recovery, and what’s being done to bring the numbers down? The research , much of it Irish-led, gives a clearer picture than the anecdotes alone.

How Common Are ACL Injuries in the GAA?

Gaelic football and hurling are officially classed alongside soccer and rugby as “Level 1” sports — the highest-risk category for ACL injury, defined by the amount of pivoting, hard-cutting, jumping, and landing involved.

The numbers back that up. A National GAA Injury Surveillance Database review covering elite football and hurling squads between 2008 and 2016 found ACL injuries accounted for roughly 1.7% of all injuries in elite football and 1.6% in elite hurling — but a separate longitudinal study found that among elite footballers specifically, 11% of all injuries were knee injuries, and ACL tears made up 13% of those. Match-play is where it happens: incidence during games was eight times higher than in training for football, and 24 times higher for hurling. Most tears happen through turning (40%), landing (26.7%), or contact with another player (33.3%) — and the vast majority (80%) are new injuries rather than a repeat tear.

The good news: recovery rates are strong

The most significant recent research came out of Dublin City University and the Royal College of Surgeons in Ireland, based at the UPMC Sports Surgery Clinic in Santry — a study following 1,891 Gaelic football, hurling, and camogie players who underwent ACL reconstruction between 2014 and 2018, with a 93.4% follow-up rate at two years post-surgery.

The headline finding: return-to-play rates were 87.8% for football and 89.5% for hurling/camogie — broadly on par with what’s reported in professional sport internationally. Of those who returned, 72.7% (football) and 76.3% (hurling/camogie) got back to an equivalent or higher level of performance than before injury. Average time to return to play was 10.8 months, with no meaningful difference between male and female players’ timelines.

“These sports are the ultimate test of knee stability,” said Professor Brian Devitt, consultant knee surgeon and one of the study’s lead researchers. “This research provides reassurance that ACL reconstruction enables a safe and reliable return to sport.”

Graft choice matters — a lot

One of the more actionable findings from the same study: the type of graft used in reconstruction made a significant difference to reinjury risk. Players who received a bone-patellar tendon-bone (BPTB) graft had a re-rupture rate of just 3.3%, compared with 15.3% for hamstring tendon grafts — a substantial gap that’s consistent with similar research from other countries. The study also found younger players and inter-county-level athletes carried the highest risk of graft reinjury, and — perhaps counterintuitively — players were twice as likely to injure the opposite knee than to re-tear the reconstructed one.

“The importance of injury prevention cannot be over-emphasised,” Devitt noted — which is exactly where the GAA’s own prevention programmes come in.

What’s being done about prevention

Two structured warm-up programmes are now widely used across Gaelic games specifically to reduce ACL and lower-limb injury risk:

  • GAA 15, developed by the GAA’s own Scientific and Welfare Committee
  • Activate, developed by Ulster GAA in partnership with Sport Northern Ireland (SINI), adapted from FIFA’s well-established “11+” programme and tailored specifically for football and hurling

The evidence for these programmes is genuinely strong. An evaluation of Activate by researchers from Ulster University and the GAA found female athletes using the programme had a 56% lower risk of ACL injury compared to those who didn’t. For context, FIFA’s original 11+ programme — which Activate was adapted from — has been shown in separate research to reduce ACL injuries by 76.5% and overall injuries by 46% when properly implemented. Given female Gaelic games players face a disproportionately higher ACL injury risk due to biomechanical factors, both the LGFA and Ladies Gaelic Football have run dedicated initiatives building similar prevention exercises directly into team warm-ups.

The catch, consistent across every injury-prevention programme studied globally: compliance is everything. Programmes only work if teams actually do them, consistently, every session — research in other sports has shown adoption can drop off sharply once the novelty wears off, even among well-resourced squads.

Where recovery support fits in

None of this changes the reality for a player facing 10+ months of rehab. Whatever the graft type or the club, the early weeks after ACL reconstruction follow a similar pattern: managing swelling, protecting the joint, and giving the muscles around the knee the best possible environment to start rebuilding strength. That’s where controlling early-stage swelling and pain — through methods like active cold and compression therapy — plays its part in a much longer road back to the pitch.
